Critical need for doctors to counsel couples on potential genetic burden in offspring of older dads

MANILA, Philippines, May 26, 2024 /PRNewswire/ -- An increasing trend of older fatherhood is potentially creating a global genetic disaster with devastating health consequences for offspring. A major conference of human reproductive health in the Philippines will hear of mounting evidence that paternal ageing is associated with a profound increase in sperm DNA damage that can burden offspring with mutational impacts including schizophrenia, autism and childhood cancers. Speaking at the 2024 Congress of the Asia Pacific Initiative on Reproduction (ASPIRE) in Manila today, Indian fertility specialist, Dr Ameet Patki, said that sperm DNA damage in older men also contributes to an increased risk of pregnancy loss and a decrease in live births. "It is imperative that clinicians advise couples where the man is aged 40 or more that there is an increased risk of adverse health outcomes in offspring, and potentially on future generations," Dr Patki said. "Over the past 50 years there has been a significant increase in the prevalence of autism spectrum disorders in children and an eight-fold increase in childhood cancers. It is reasonable to assume that advanced paternal age is a contributor to this disturbing global trend. "The average age of fathers is increasing, as it is among mothers as couples delay parenthood because of economic, social and lifestyle issues, and sperm counts in men in many countries are declining at an accelerated pace. "We know that sperm count, volume and motility decrease with age, but the focus should also be on genomic defects in sperm." Dr Patki, who is President of the Indian Society for Assisted Reproduction (ISAR), said there had been major international studies on the impacts of maternal ageing and reproductive health, but research and clinical trials on paternal ageing were scant by comparison. "In many societies, infertility is perceived as a female issue and many men are unwilling to accept that they may be a contributor to adverse reproductive outcomes," he said. "However, the biological clock also ticks for men particularly in terms of sperm DNA fragmentation that occurs exponentially after the age of 40. "Paternal ageing is associated with a profound increase in sperm DNA damage, the appearance of multiple epigenetic changes in the germ line, and an elevated mutational load in the offspring. "The net result of such changes is an increase in the disease burden carried by the male and female progeny of ageing men including autism, attention deficit disorder and schizophrenia. "The impacts of male biological time keeping are entirely different than maternal ageing. "Genetic counselling should be considered for couples with advanced paternal ageing along with testing for sperm DNA fragmentation, particularly in cases of recurrent pregnancy loss." Dr Patki said treatment with antioxidants, such as vitamins E and C and CQ10, could improve sperm DNA integrity and enhance semen parameters in men above the age of 40. "Carefully designed clinical trials are now needed if we are to realise the therapeutic potential of this approach," he explained. Meanwhile, a major focus of the Fertility Counts initiative being advanced by ASPIRE at the Manila Congress is that raising a family at a younger age is medically desirable, while understanding and addressing social and economic barriers to couples seeking parenthood in their prime reproductive years. This is an important role of fertility counsellors, particularly those providing support for couples who are struggling in their treatment journeys, those with guarded reproductive prognoses, or those who have had unsuccessful treatment cycles. Ms Kerner is Chair of the ASPIRE Special Interest Group of Psychology and Counselling and President of the Australian and New Zealand Infertility Counsellors Association. MANILA, Philippines, May 24, 2024 /PRNewswire/ -- Millions of women around the world continue to suffer unnecessarily from the physical and psychological effects of the menopause because of "mistaken fears" about the risks and benefits of hormone replacement therapy. A world leader in human fertility and women's health has warned that continued under-utilisation of menopausal hormone therapy (MHT) will result in growing numbers of women experiencing diminished quality of life because of cardiovascular and metabolic diseases, osteoporosis and premature cognitive decline. Addressing a global conference on human fertility in the Philippines today, Professor Eileen Manalo said more women now have a life expectancy exceeding 80 years meaning many will spend over a third of their lives in a postmenopausal state driven by hormone deficiency. "Up to 80 per cent of all women are affected by menopausal symptoms with a quarter describing them as severe," she said. "Although the menopause is a natural transition, it has important physiological manifestations resulting from hormonal changes that have far reaching short and long-term consequences. "There is robust evidence that MHT initiated before the age of 60, or within 10 years from when menstruation ceases, is a highly effective treatment for the symptoms of the menopause, that it prevents hip fractures and reduces cardiovascular disease mortality." Professor Manalo said MHT had been "demonised" over two decades ago by a study of thousands of women taking the hormone therapy that reported it was associated with higher risks of breast cancer and stroke. She said countless women abruptly stopped MHT and many health care providers became reluctant to prescribe the therapy. Eileen Manalo is a Professor in the Department of Obstetrics and Gynaecology of the University of the Philippines College of Medicine. She is a former President of the Philippine Society for Reproductive Medicine and is currently Assistant Secretary General of the International Federation of Fertility Societies. Speaking at the 2024 Congress of the Asia Pacific Initiative on Reproduction (ASPIRE) in Manila, Professor Manalo said the report published 22 years ago had devastating consequences. She said a legacy of the report had been a fear of cancer among women that had overshadowed the potential benefits of MHT, which have been informed by more recent randomised control studies (RCTs) around the world. "Cumulated RCT findings are consistent with the large body of observational studies that consistently show a reduction of all-cause mortality and cardiovascular disease in women who initiate hormone replacement therapy at the time of menopause," Professor Manalo explained. "Additional MHT benefits include reduction in menopausal symptoms, potential reduction in cancer and other mortalities, prevention of new onset diabetes mellitus and osteoporosis with increased bone fracture prevention and improved quality of life. "Evidence-based data from RCTs are reassuring that risks associated with MHT are rare – less than 10 cases per 10,000 women – when initiated at 60 years or less or within 10 years on menopause onset. "The magnitude and types of MHT risks, including breast cancer, stroke and cardiovascular disease (CVD) are rare. "They are not unique to MHT and are comparable with or less than other commonly used medications including those used for CVD prevention, such as statins, aspirin and other regular medications used for high blood pressure, for example ACE inhibitors. "Meanwhile, an increase in stroke and cardiac deaths has been reported among women who discontinued their MHT compared to women who went on to use hormone therapy. "The past 22 years since the report have brought a better understanding of the overall risks and benefits of MHT. "It is time to heed the call of the International Federation of Gynaecology and Obstetrics for healthcare practitioners to protect the lives of mid-life women by increasing awareness of the symptoms of menopause, providing healthcare options including MHT, and promoting healthy lifestyle changes. (Refer to Annex B for details on CESG) Singapore International Water Week (SIWW) Key themes that will be presented at SIWW2024 include water sustainability, net zero and decarbonisation, resource circularity and digitalisation. A new pillar on climate adaptation, specifically focused on coastal protection and flood resilience will be introduced at SIWW2024. SIWW2024 will also feature an Industrial Water Solutions Forum, a technology and innovation platform targeted at industrial water users. Positioned as a global show with a strong relevance to Asia, SIWW2024 will receive business delegations from Malaysia, Vietnam, Thailand, Indonesia, India, among others who will source for solutions in the Water Expo. In cooperation with IFAT and organised by Messe Munich, the SIWW2024 Water Expo will present over 400 exhibitors across three days. (Refer to Annex C for SIWW2024 content line-up and list of Water Expo exhibitors) The Water Expo will showcase global and cutting-edge equipment, technology and solutions from over 30 countries and regions. Key highlights at this year's Water Expo also include three thematic zones – a new "Coastal Protection and Flood Resilience" zone, a "Start-Up" zone, and the "Smart Tech & Digitalisation" zone. New coastal protection research projects and applied research grant calls  Minister Grace Fu will also be speaking at the Coastal and Flood Resilience Leaders Summit on 20 June. The Summit will bring together industry and city leaders to discuss case studies and applied solutions in building climate-resilient cities. In her speech, Minister Fu will announce that the Coastal Protection and Flood Resilience Institute (CFI) Singapore will be commencing eight new projects under the Coastal Protection and Flood Management Research Programme (CFRP). She will also announce a new applied research grant call under the CFRP. Largest ever gathering of utility and city leaders at SIWW2024 SIWW2024 will host the largest ever gathering of CEOs and senior representatives from water agencies, utilities, and cities. Close to 100 global water utilities and water agencies, including more than 50 CEOs, will be attending the Utilities CEO Roundtable, and participating in high-level discussions and plenaries on key and emerging topics such as water circularity and net zero. Collectively, these utilities cover major urban centres around the world including California, Tokyo, Dhaka, Sao Paolo, Chennai, Johannesburg, Sydney, Manila and Shenzhen. Cherry Award is named in honor of William R. Cherry, a founder of the photovoltaic community. The purpose of the award is to recognize an individual engineer or scientist who devoted a part of their professional life to the advancement of the science and technology of photovoltaic energy conversion.  Dr. Xu Xixiang is currently leading an R&D team over 500 technical staff members working on multiple PV technologies. He led the team and created 14 world records of Si solar cell efficiency and now holds efficiency records of HJT, HBC and PVSK/HJT tandem solar cells, which are 26.81%, 27.3% and 34.6%, respectively. LONGi is not only the creator of the world record for solar cell efficiency, but also a leader in promoting innovation in the global photovoltaic industry.
