According to the modern medical science, the substances in the amniotic fluid including neonatal meconium, bilirubin, fetal fat, fetal hair, epithelial cells etc. are so-called fetotoxics. Neonatal jaundice
The jaundice of baby less than one month old (within 28 days after birth) is called as neonatal jaundice. The neonatal jaundice is characterized as such symptoms like skin, mucosa and sclera jaundices caused by raised blood bilirubin level due to abnormal bilirubin metabolism during the neonatal period, also called as neonatal hyperbilirubinemia.
The metabolic process of bilirubin is that the indirect bilirubin is converted by the liver into direct bilirubin, and discharged into the intestine with the bile, and most is excreted with the feces, while the small part is absorbed with enterohepatic circulation and returns to the liver again. Most of the bilirubin which is re-absorbed and returned to the liver is discharged at its original form to the intestine with the bile again, and the small part join the circulation inside the body.
Because the fetus in the mother body cannot excrete the feces, the bilirubin discharged by the fetus into the intestine is repeatedly absorbed under the enterohepatic physiological circulation, resulting in too high bilirubin level (80- 100mg) in the body of fetus after birth. Meanwhile, the repeated absorption of bilirubin in meconium caused by the enterohepatic physiological circulation gives more burdens on the liver, causing bilirubin metabolism dysfunction, and thus leading to physiological or pathological neonatal jaundice.
Neonatal jaundice and meconium
In the neonatal meconium, in addition to large quantity of fetotoxic substances, the bilirubin level is extremely high, about 80-100mg, equivalent to 5-10 times the amount of bilirubin generated in neonates daily, therefore the meconium is in dark green (black) color, If the meconium is excreted slowly, the bilirubin shall be absorbed too much in the intestine, this will generate neonatal jaundice or aggravate neonatal jaundice.
