1.Co2 laser treatment principle for fungus nail
Co2 laser burn the lesion nail by using the Photothermal principle,make the lesion tissue heating, gasification, decomposition
On the one hand, CO2 laser temperature is high, the place of fungal disease nail can directly kill, gasification.On the other hand it’s easy for finger nail trimming after the treatment of Co2 laser to lesion nails.
2.The advantages of Co2 laser use in the treatment of onychomycosis :
(1).Method is easy ,low cost,patients had good compliance and no complication or substitute;
(2).It Can substitute part internal and it’s an ideal method for the treatment of onychomycosis;
(3).Particularly suitable for whom has liver and kidney function damage as well as to the patients who has psychological disorders to medicine.
3.Treatment Method
.Method:
² Making a record for the clinical type to the lesion toenail before the treatment;The number of affected nails.
² Measuring the longitudinal diameter and diameter of the lesion nail bed by using 5 times the magnifying glass and observe the the degree of the affected nails.
² Co2 treatment for one time every 2 weeks and record the change of the lesion toenail every time.
² Choose Co2 laser Wavelength:10600nm;Output power:10-15W; The diameter of the focus of the laser light spot:0.4mm;Pulse time:0.01-0.99s;Distance to the toenail about 1cm;Aim the laser beam to the lesion toenail.
² Burning a hole in sequence evenly, each hole diameter is 1 mm, each hole spacing is 0.5 mm.Depth should be depending on the thickness of the lesion nails.
² Just through the lesion nails to the nail bed ,it’s ok.When having slight burning sensation,it’ll be ok.But must pay attention to it:the laser can’t hurt the nail matrix,so as not to affect the growth of the deck.
² The diameter on the surface of the punch should beyond the lesions deck for 2-3mm.Asking patient cutting their nails properly.To stimulate new nail growth.
² After a fully cure for affected nail,we can stop treatment.
² After treatment for 3 months and 6 months,we should observe the growth and recurrence situation of lesion toenail.
4.The standard judgment for treatment effect
² Fully cured:Deck is smooth,flat,gloosy, fungi direct microscopy negative.
² Remarkable result:The lesion toenail fade≥70%,the fungus direct microscopy negative or positive.
² Improvement for lesion toenail:30%-69%,the fungus direct microscopy negative or positive.
² A fading is invalid for the lesion toenail:<30%,the fungus direct microscopy negative or positive.
5.Results:
Clinical effects:
Nail fungus disease and toenail fungus disease 12 weeks of recovery rate for:26% and 16%;24 weeks to heal at a rate of 80% and 80%
In a total, 12 weeks of onychomycosis treatment effective rate was 57%, 24 weeks with efficiency of 90%.(As shown in table 1, 2).Has the remarkable difference after statistics processing. χ2=7.55,P<0.01.DLSO and SWO,a type of different disease cure rate was no significant difference,χ2=0.55,P>0.05
